fIZZY fORM aka Jack Nickles (real name knowoneknows, born 1981 in Salt Lake City, Utah) is an american independent underground hip hop artist/producer.
fIZZY fORM has a series of underground releases called Knowoneknows, mixed by The 45 fELON. He has also appeared on internet radio, college radio, and local hip hop radio shows. He has been featured on a list of releases and is most known for Salt Lake City's own anthem "It's the Lake". His "I Represent" mentality has earned him the opportunity to collaborate with some of his most influential artists during his musical progression… Cali Agents (Planet Asia and Rasco), Copywrite (MHz), and Sadat X (Brand Nubian), to name a few.
